The form you will complete creates the service agreement between you/participant and Luvme. This is made for the purpose of providing supports under the Participant’s National Disability Insurance Scheme (NDIS) plan.
The NDIS is a scheme that aims to:
- Support the independence and social and economic participation of people with disability, and
- Enable people with a disability to exercise choice and control in the pursuit of their goals and the planning and delivery of their supports.
You are a self-managed NDIS participant, place an order and you will be emailed an invoice you can claim payment on from the NDIS.
If you are a plan-managed participant, place your order and provide us with the email address of your plan manager when you fill out the form at the end of the ordering process. We’ll send the invoices to them for processing.
If you are an agency-managed participant, place your order and provide your plan details by filling out the form prompted at the end of the ordering process.

As an NDIS participant we need to know some information the first time you order. When you order again just choose NDIS as the payment method during checkout and we will use this same information again.
